The modern approach to stress urinary incontinence with the FemiLift laser

The procedure using the FemiLift Fractional CO₂ laser results in the contraction of collagen and elastin fibres and induces the production of new collagen fibres, which strengthen the support of the middle urethra. This treatment does not cause aggressive ablation and leads to faster tissue healing. The FemiLift System, used in SKINMED®, addresses the entire periurethral space, increasing the thickness of the vaginal walls and thus providing the middle urethra with superior support.

Before treatment, a gynaecological examination of the patient takes place to rule out vaginal bleeding, vaginal infection or cervical abnormalities. The patient must also have a negative Pap smear.

How is urinary incontinence treatment with the FemiLift fractional CO₂ laser performed?

The treatment is performed with a cylindrical laser probe, the FemiLift probe, which is inserted into the vagina (it mimics the shape of the vagina to ensure patient comfort) to be positioned under the middle urethral area and rotated 120 degrees. Three treatment sessions are carried out at 4-6 week intervals, and one more session after 6 months; each session lasts about 10 minutes, during which time 3 passes of the probe are made.

This type of laser treats mild to moderate urinary incontinence and relieves symptoms in cases of severe SUI (stress urinary incontinence). Some studies have reported an improvement in urinary incontinence symptoms in 80%-90% patients.

Vaginal rejuvenation with the FemiLift laser

Laser energy, applied to the tissues of the genital area, penetrates and improves the quality of the mucosa or skin by stimulating the synthesis of collagen, elastin and increasing vascularisation. Treated tissues will be firmer, more elastic, more resistant. By stimulating the synthesis of collagen and elastin, a remodelling treatment of the basal layers of the vaginal mucosa is achieved, with several advantages:

  • narrowing of the vaginal canal and increased tone of the vaginal wall, with the effect of increasing sexual satisfaction for both patient and partner.
  • increased elasticity and lubrication of the genital mucosa, with the effect of disappearing or relieving discomfort spontaneously or during sexual contact.
  • relief of mild to moderate urinary incontinence.

By increasing the quality of the vaginal mucosa and stimulating local immune processes, beneficial effects can be seen in chronic vaginitis and HPV infection, as some studies indicate.

The FemiLift laser can also be used to treat the vaginal introitus (opening of the vagina) and the vulvar area (labia majora, labia minora, clitoris), for the treatment of lichen sclerosus, tissue laxity, mucosal atrophy, dryness of the genital mucosa.

How to perform laser vaginal rejuvenation in SKINMED®?

Before treatment, a gynaecological consultation is required, with a Pap smear and vaginal secretion cultures.

For best results, 3 sessions are recommended, performed every 4 weeks, plus another session after 6 months. 1-2 maintenance sessions per year can be performed.

What is urinary incontinence?

Urinary incontinence is a public health problem as it is estimated to affect 200 million people worldwide. Urinary incontinence can be defined as the involuntary loss of urine.

Depending on aetiology and symptoms, urinary incontinence has been divided into four main classes:

  • stress urinary incontinence
  • urge incontinence
  • mixed incontinence (a combination of the first two)
  • incontinence by "too full".

What is stress urinary incontinence (SUI)?

Stress urinary incontinence (SUI) is characterised by loss of urine with physical exertion, coughing, sneezing, laughing, exercise or even when changing body position or with minimal exertion when there is severe damage to the urethral sphincter.

It has been further divided into incontinence caused by hypermobility of the urethra (the urethra changes position when abdominal pressure increases) or incontinence caused by urethral sphincter defect.

Among the main factors involved in the development of SUI are:

  • pregnancy and vaginal birth (instrumental delivery by forceps seems to increase the risk of urinary incontinence the most),
  • menopause
  • obesity
  • chronic cough in smokers or those with lung disease
  • nerve and muscle damage following gynaecological or pelvic surgery.

A study of a large number of cases estimated the prevalence of urinary incontinence at 30% for women aged 30 to 60, and about half of these cases are attributed to SUI.

On average, a woman waits 6.5 years from the time she first experiences symptoms to see a doctor and get a diagnosis. The importance of this condition is that even mild symptoms affect a woman's professional, personal, sexual and social life. These patients often struggle with daily activities such as shopping or going to a movie because of the fear of not being around a toilet.

How does the FemiLift CO₂ laser work to treat incontinence?

A modern treatment method that has relatively recently entered the therapeutic arsenal for stress urinary incontinence is the FemiLift fractional CO₂ laser.

The laser wave is delivered through an optical element that splits the beam into 81 microscopic pixels; this pixelated pattern does not cause aggressive ablation and leads to faster tissue healing because the microscopic beams penetrate the submucosa where they exert their deep thermal effect, leaving the surrounding tissue intact with minimal ablation. The procedure leads to contraction of collagen and elastin fibres and induces the production of new collagen fibres, which strengthen the support of the middle urethra. The Femilift System addresses the entire periurethral space, increasing the thickness of the vaginal walls and thus providing the middle urethra with superior support.

What side effects and post-procedure indications are there for urinary incontinence treatment?

The rate of adverse reactions is extremely low. The procedure is virtually painless (it is performed without anaesthesia), quick, safe and does not require interruption of daily activities. The patient may feel slight discomfort 2-3 days after the procedure and should avoid sexual contact for 5 days.

When is vaginal rejuvenation treatment indicated?

This type of treatment addresses:

  • Patients with vaginal laxity after pregnancy, natural childbirth, episiotomy or other causes who have noticed a decrease in sexual satisfaction.
  • Patients with urogenital menopausal syndrome (dryness, burning, atrophy, lack of lubrication of genital mucosa, painful sexual contact).
  • Patients who - due to age, hormones, pregnancy - notice unsightly changes in the vulvar area (laxity, pigmentation, atrophy, dryness).
  • Patients with chronic genital lichen sclerosus.
  • Patients with mild/moderate urinary incontinence.

What side effects and post-procedure indications are there for vaginal rejuvenation treatment?

The vaginal rejuvenation procedure is painless, is not performed under anaesthesia, does not require interruption of daily activities, but it is recommended to avoid sexual contact for 5 days.

The patient may feel tenderness or slight discomfort 2-3 days after the procedure, but no medication is required after the procedure.
